You can formally request your medical records in writing. You may wish to do this if: The information you need is not covered by your GP record You want hard copies of records
How to request a duplicate or replacement license?
You must provide:
- DOT Medical Card and two proofs of domicile in Tennessee
- Confirmation of proof of identity and
- Required Documentation of changes (other than those needed to correct clerical errors) in a license, temporary license or certificate for driving holder’s name, date of birth, or gender.

Are you legally entitled to copy of your medical records?
You have every legal right to request a copy of any or all of your medical records. It is permissible to charge a reasonable fee to cover the cost of providing those records but the fee should be minimal. I always give every patient a copy of their audiogram for their own records after we explain the results to them.
